2012 FLL® Event Forms and Instructions
Team Information Sheet
For each event, each team needs a team photo to complete four copies of the
2012 FLL Team Information Sheet (PDF with fields to fill in) (also known as the Profile form)
Give copies to (1) Emcee or Head Referee at your Practice Round, (2) Robot Design judges, (3) Project judges and (4) Core Values judges
FIRST Consent and Release Agreement
For each event, each team participant needs to complete and sign the
2012 FIRST Consent and Release Agreement or the
2012 FIRST Consent and Release Agreement - Spanish
Student forms need to be signed by a parent or guardian. Turn in forms at the team registration table when your team checks in.
Coach's Promise
For each event, the coaches need to sign the
2012 FLL Coach's Promise
Turn in forms at the team registration table when your team checks in.

Judging Rubrics

Core Values Poster (CVP)
We will not be using the Core Values Poster this year at Los Angeles Region FLL Tournaments.
Robot Design Executive Summary (RDES)
We will be using the RDES this year at Los Angeles Region FLL qualifying and championchip tournaments.
Teams are expected to spend up to three hours preparing a presentation up to four minutes long.

Teamwork Exercise
A teamwork exercise will not be used during Core Values judging at Qualifying Tournaments, but it will be used at the Championship Tournaments.
